PERSONAL SERVICES CONTRACT <br /> BETWEEN: The City of Eugene, an Oregon <br /> Municipal Corporation (City) <br /> AND: John O'Connor & Paul Hoobyar (Contractor) <br /> CONTRACT NO.: 98 -00404 <br /> RECITALS <br /> A. Contractor is engaged in the business of Meeting /Groups Facilitation Services. <br /> B. City desires to engage Contractor to provide the services described in this agreement and <br /> Contractor is willing to provide such services on the terms and conditions set forth herein. <br /> C. The following are attached and incorporated as a part of this contract: <br /> Exhibit A - City of Eugene Standard Contract Provisions <br /> Exhibit B - Vendor's Proposal & Payment Schedule <br /> AGREEMENT <br /> 1. Services to be Provided. Contractor agrees to provide to City the following services ( "the Work "): <br /> Provide meeting facilitation services at meetings of the Hendricks Park Departmental Advisory <br /> Committee for the Maintenance Division of the Public Works Department. Facilitation will be <br /> provided by a combination of John O'Connor and Paul Hoobyar. John O'Connor shall be designated <br /> as the lead facilitator. Details for the scope of services and agreed rates of payment are shown in <br /> Exhibit "b" attached "Hendricks Park Meeting Facilitation Proposal" dated January 22, 1998 <br /> addressed to Johnny Medlin from John O'Connor and Paul Hoobyar. <br /> 2. Time for Completion. Contractor shall commence the Work on March 1,1998, and shall complete <br /> the Work prior to March 1, 1999. <br /> 3. Consideration. The consideration which City shall pay to Contractor, for both the Work performed <br /> by Contractor and the expenses incurred by Contractor in performing the Work, shall not exceed <br /> $5,000 based on the schedule of rates described in the attached Exhibit B. City shall make <br /> progress payments, based on monthly invoices from Contractor, together with a progress report <br /> containing a summary of the Work completed for which payment is requested. City's payment <br /> shall be made within 30 days after Contractor's statement. Contractor shall not be entitled to <br /> reimbursement for travel or other expenses unless specified on the attached schedule of rates. If <br /> Contractor is entitled to reimbursement for any such expenses, they shall be properly documented <br /> and submitted in accordance with City requirements. <br /> Personal Services Contract- -Page 1 <br /> (Revised December 1997) <br />
