r Policies and Guidelines for the Community Use EUGENE of Artificial Surface Facilities 7.1 Application Process Applications are processed annually, beginning on the following dates: Reservation Period Processing Date Fall (Third Monday in August - November 30) June 1 Winter (December 1 - Third Monday in February) October 1 Spring (Third Monday in February - May 31) December 1 Summer (June 1 - Third Monday in August) March 1 Permit applications may be made in person at: City of Eugene Athletics Office, Amazon Community Center, 2700 Hilyard Street , Eugene, OR 97405. Office hours are Monday- Friday, 9:00 a.m. - 6:00 p.m. Phone: (541) 682 -5409 Fax: (541)682 -5367 Application Fee $5.00 per application - General Rentals $25.00 per application - Special Events Application Fees are not refundable Facility Deposit $50.00 deposit - General Use Permit $100.00 deposit - Special Use Permit Reservations will not be confirmed until the deposit is paid. The deposit will be refunded under the following conditions: 1. Cancellation by applicant occurs more than three (3) weeks prior to reservation. 2. The facility is left in good condition and is left undamaged (see requirements below). 3. Cancellation occurs due to actions of the City. Rental Fees School District 4J rental rates apply to the use of the fields. They are as follows: Class I User - No charge Class IIa User - $15.00 per hour Class II User- $20.00 per hour Class III User - $40.00 per hour Light Fee - $20.00 per hour (September - November) $30.00 per hour (December - August) All fees must be paid in full at least three (3) working days prior to the event/activity. Approved youth groups are exempt from rental and Tight fees. School District 4J maintenance fees are charged to all approved youth groups. -1-
