Federal Emergency Management Agency E- Grants Page 4 of 10 <br /> Scope of Work: PA- 10 -OR- 4169 -PW- 00025(0): <br /> WORK COMPLETED <br /> The applicant utilized its force account labor and equipment to repair the <br /> street lights(27) along with the damaged road signs (45). <br /> The applicant used (238 reg force account labor and 27.4 OT hrs.) of <br /> personnel for a total of 265.4 hours at a cost of $9,686.90 for reg labor <br /> cost and $ 1,004.55 for OT. <br /> Applicant used force account equipment for a total of 111.4 hours at a <br /> cost of $4,599.91. <br /> Applicant affirms that none of this claim is covered by insurance. <br /> SCOPE NOTES <br /> Equipment hrs. are less than personnel because work was primarily <br /> performed in 2- person teams per 1 piece of equipment. <br /> The applicant has not been able to provide any pictures. <br /> PERMITS: The applicant shall obtain and document compliance for any <br /> required coordination, permits, and /or consultation with applicable <br /> regulatory agencies. This documentation may be required for closeout. <br /> Failure to do so may jeopardize Public Assistance funds. <br /> RECORD RETENTION: The State of Oregon requires applicants to <br /> maintain all project related records for a period of six (6) years from <br /> applicant closure which also satisfies the federal (44 CFR §13.42) <br /> records retention requirements. <br /> DIRECT ADMINISTRATIVE COSTS: The applicant is not requesting <br /> Direct Administrative Costs that are directly chargeable to this specific <br /> project. <br /> MITIGATION: Mitigation was discussed with the applicant; no feasible <br /> mitigation opportunities were identified. <br /> EQUIPMENT RATES: The public works dept is using their own <br /> equipment rates which are less is than the FEMA rates. <br /> Hazard Mitigation Proposal <br /> Is effective mitigation feasible on this site?
