The form can be obtained via the Internet at the following URL: <br /> http://www.dnb.com/dunsno/dunsno.htm <br /> USING THE D -U -N -S NUMBER <br /> Grantees must provide their D -U -N -S Number (and their TIN) when applying for grants. Grantees and <br /> payees use D -U -N -S Numbers as identifiers when contacting the Department of Education about <br /> payment requests or grant awards. <br /> CHANGES TO THE D -U -N -S NUMBER <br /> The D -U -N -S Number represents the payee's institution as a financial entity. The number must change if <br /> the payee's organization: <br /> • Merges with another organization, <br /> • Is sold to another organization, or <br /> • Separates from an existing organization and becomes a freestanding financial account on its own <br /> right. <br /> Report name and address changes to Dun & Bradstreet at 1- 800 - 234 -3867.
